Diabetes Disease Management. DIADEM?s overall objective is to establish Diabetes Disease Management Services at European level as a platform for improved patient care and a more effective health care delivery. This will combine the efforts of GPs, medical reference centres, insurer, support centre and IT industry to serve the diabetic patients towards a better long-term management of their disease. Secure, transparent and shared access to patient-related data are at the core of the service. DIADEM test sites starting in United Kingdom and Germany are in view of further development in other European countries, followed by an international roll-out.
